Form W-9

The information on this page may be relevant to the following types of Securityholders:

• United States citizen
• Individuals who are residents of the United States for US tax purposes

If a Registered Securityholder has determined that they are required to complete Form W-9 only, they are encouraged to complete the form online.

To complete the form online, follow the steps below:

• Login by entering the Username, Password and PIN previously sent to Securityholders.
• Enter various details to complete the form.

If a Registered Securityholder would prefer to complete a paper copy of the form, they should contact the BBI Securityholder Infoline to request a copy of the paper form.
